Forms and Maps

If your child/youth wants to attend any off campus events with Abundant Life Assembly of God, they must have a completed form on file with either the Children’s Ministry department or Youth Ministry department. Download the form and turn it in to Pastor James Logan (for children’s and middle school ministries) or Pastor Ben Skinner (for youth ministries).  You can also mail the form to the church office.
